The death of a construction worker who fell 45 feet while performing repairs on a Philadelphia church roof in June has led to 10 safety violation citations, including three willful citations, against James J. McCullagh Roofing Co. in Jenkintown.
The U.S. Labor Department’s Occupational Safety and Health Administration cited the company with failure to provide adequate fall protections and fined it $71,600 in penalties, which it can appeal.
Violations are considered willful if the employer knows death or serious injury could result from a known hazard. The company’s listed phone number is not working.
“This tragic accident could have been prevented had the company provided the proper fall protection for employees,” said Domenick Salvatore, director of OSHA’s Philadelphia Area Office. – Jane M. Von Bergen
